Larson Apparel Request Form

Fill out the form below to place your Larson Apparel order. Orders will be placed on a quarterly basis if minimum quantities are met. 

This program is separate from the Larson Uniform Program for branch employees who are required to wear a uniform.

Payment for Larson Apparel will be done via a payroll deduction. Please fill out the Payroll Deduction Acknowledgement form and return to Lisa Rick ( Your order will not be placed until we have this signed form on file.

Due to supply chain challenges, please allow at least 6 weeks for processing once the order has been placed with our vendor. Larson Apparel items will be shipped directly to the branch.

If you need to order more than two separate apparel items, please submit the form again. 

Please refer to the Symbol Key on the Larson Apparel Options PDF to determine available colors, Tall and Women’s companion sizes.
